GladdMD initial consultation deposit

This deposit is collected prior to or at the time of scheduling an initial consultation with GladdMD. This is a requirement for scheduling and secures your spot on our calendar.

Terms: This deposit will be applied towards your initial consultation at the time of check out on the date of service. This deposit is non-refundable for cancellations and no shows.
